Improving Advance Care Planning processes in your Residential Aged Care Facility
Why digitise residents ACPs?
An Advance Care Plan can only guide a person’s care if the right people can access it when they need it.
For residents of aged care facilities, this becomes particularly important when they are transferred between services, including hospital, palliative care and other healthcare settings.
Palliative Care ACT invites Residential Aged Care Facility managers and other relevant senior staff to a practical morning exploring how we can work together to ensure Advance Care Planning documents are digitised, up to date and accessible through My Digital Health Record (MyDHR).
What we’ll cover
Paul Arztenhofer (CEO - Palliative Care ACT) and Janine Rath (Advance Care Planning - CHS) will lead a one-hour presentation exploring:
- why accessible Advance Care Planning documentation matters
- the role of digitised Advance Care Plans when residents move between care settings
- the importance of uploading relevant documents to My DHR
- practical considerations for Residential Aged Care Facilities
- how better access to these documents can support residents, families and healthcare teams when important decisions need to be made.
We also hope to share lived-experience perspectives illustrating what can happen when Advance Care Planning information does, or doesn’t, travel with a person as their care needs change.
The presentation will be followed by an hour of networking and refreshments, with an opportunity to continue the conversation with colleagues from across the residential aged care sector.
Palliative Care ACT is also exploring whether our volunteers may be able to assist facilities with the practical task of digitising Advance Care Planning documents. This event will help us better understand what support facilities may need and where PCACT may be able to help.

This event is designed for Residential Aged Care Facility managers and other senior staff responsible for resident care, clinical governance or Advance Care Planning processes.
